ELKADER FITNESS INC.
MEMBERSHIP AGREEMENT
It is agreed by and between Elkader Fitness Inc. (Club) and ______________________
(Member) as follows:
Member hereby agrees to be a member of the Club for a term of one (1) year beginning on the date of this contract.
II. FEES
Member agrees to pay membership fees as checked on the fee schedule below. The first payment is due on the signing of this contract, receipt of which is acknowledged by the Club.

Couple ___$420.00 ___$215.00 ___$117.00
Family ___$480.00 ___$245.00 ___$132.00
Additional semiannual payments shall be due six (6) months from the date of this contract.
Additional quarterly payments shall be due three (3) months, six (6) months and nine (9) months from the date of this contract.
III. MEMBERSHIP RIGHTS
Members shall have voting rights at the annual meeting and any special meetings of the members of the Corporation and shall be eligible to serve on the Board of Directors and other rights and responsibilities as set out in the by-laws of Elkader Fitness Inc.
Members shall have full use of the Club’s facilities and equipment along with any Club sponsored activities for the one (1) year term of this agreement. Access to the building will be a coded password and members’ use of the building shall be unsupervised. Member agrees to keep any passwords, combinations or other entry information completely confidential and shall not share such information with any other persons.
V. LIABILITY WAIVER
Member agrees and understands that the use of the Club’s facilities and equipment will be unsupervised and the member agrees to assume any risk for member’s use of said facilities and equipment. Member agrees to sign the Waiver and Release Form attached to this agreement which shall become a part hereof.
